Consist of 2 appointments, one 90 min appointment and a 30 min follow up appointment.

Full History Intake, of medications, supplements, allergies, past medical history, concerns, lifestyle, and goals. Follow up appointment will be 1 week later to go over recommendations from initial consultation.

Follow-Up of Previous Consultation (for established patients only), to discuss any changes or concerns since last consultation with review of labs with revamping of recommendations if needed.

Includes a total of 4 visits, consisting of one (1) 90 min consult, and three (3) follow up 45 min consults. Essentially one visit per week for the month. This is helpful if you have more complex health concerns and need more upfront care.

Please allow 48 hours notice prior to visit for cancellations.

Appointments cancelled less than 48 hours notice will incur 50% of the visit cost, unless there was an emergency, such as serious illness, family emergency, or inclement weather.

Missed appointments without any notification will incur the full visit cost.

In order to ensure your appointment, a valid credit card must be provided. Your card will be stored on a secure server and will not be charged to hold the appointment. In the event of a missed appointment or late cancellation, your card will be charged accordingly. If you do not wish to leave a card on file, you may contact the office for alternative arrangements.

Once your appointment is scheduled, you will receive a patient portal invitation and your intake paperwork.

Due to Naturopathic Doctors not being licensed in the state of Florida, we can not accept insurance, therefore cash/credit is the only method of payment at this time.
